Non-surgical Fotona body laser treatment combining Nd:YAG and Er:YAG modes for fat reduction, skin tightening, and cellulite (OrangeLase™). Treats abdomen, flanks, thighs, arms, and more. From £650 per session.
TightSculpting Body Contouring & Cellulite Reduction at Centre for Surgery, London
TightSculpting in summary: A non-surgical body laser treatment using Fotona’s dual-wavelength Nd:YAG PIANO® and Er:YAG SMOOTH® laser modes to reduce stubborn fat, tighten loose skin, and improve cellulite — all in the same session. Performed in 30–60 minute sessions. From £650 per session at Centre for Surgery, with course-of-3 packages from £1,750. CQC-regulated clinic, GMC-registered practitioners, 0% APR finance available subject to status.
TightSculpting uses two complementary laser wavelengths in sequence. The Nd:YAG laser in PIANO® mode delivers deep, controlled heat to the subcutaneous fat layer, causing fat cell breakdown and natural elimination by the body’s lymphatic system. The Er:YAG laser in SMOOTH® mode then tightens the overlying skin by stimulating new collagen and elastin production. The combined effect is meaningful body contouring without surgery, incisions, or general anaesthesia.
What TightSculpting treats:
For patients with significant volumes of stubborn fat, surgical or will produce more dramatic results in a single procedure. For more aggressive skin tightening, may give a more pronounced result. Your practitioner will tell you directly at consultation which path suits your case.

What is TightSculpting?
TightSculpting is a non-surgical body laser treatment developed by Fotona using two complementary laser wavelengths from the Fotona SP Dynamis platform — Nd:YAG and Er:YAG — applied in sequence to the same treatment area to reduce fat, tighten skin, and improve cellulite.
The treatment combines two Fotona laser modes in a structured two-step protocol:
Step 1 — Nd:YAG PIANO® mode (deep heating). The Nd:YAG laser in long-pulse PIANO® mode delivers homogeneous, controlled heat to the subcutaneous fat layer beneath the skin. The 1064nm Nd:YAG wavelength penetrates deep into the dermis and into the adipose (fat) tissue, raising the local temperature into a range that disrupts fat cell membranes. The body’s lymphatic system then naturally clears the disrupted fat cells over the following weeks. This deep heating also begins the collagen contraction process in the dermis above.
Step 2 — Er:YAG SMOOTH® mode (surface tightening). The Er:YAG laser in non-ablative SMOOTH® mode then delivers controlled heat to the dermis without breaking the skin surface. This stimulates immediate collagen contraction and longer-term remodelling of new collagen and elastin fibres. The result is firmer, tighter skin overlying the treated fat layer.
Standalone fat-reduction treatments (cryolipolysis, ultrasound) often leave loose skin behind after fat is removed. Standalone skin-tightening treatments (RF microneedling, non-ablative laser) don’t address fat. TightSculpting solves both problems in the same session — the fat is reduced and the overlying skin tightens to match the new contour. This is what makes it particularly effective for post-pregnancy abdominal laxity, post-weight-loss skin laxity, and cellulite (where both fat layer disruption and dermal tightening are needed).
TightSculpting isn’t a weight-loss treatment. Patients who want significant overall fat volume reduction need surgical or . It also doesn’t address pronounced skin laxity (e.g. post-massive-weight-loss aprons or significant skin redundancy) — those cases need surgical excision (tummy tuck, arm lift, thigh lift). And it doesn’t reposition tissue anatomically the way a facelift or neck lift does for the face.
TightSculpting Before & After Photos
The cases below show real TightSculpting results from patients treated at Centre for Surgery. We only publish photographs where the patient has provided written consent. A wider range of cases is available to view in person at consultation. Cellulite-specific before/after photos are in Block 8 below.
Setting realistic expectations. TightSculpting produces gradual, layered improvement over a course of treatments. Most patients see initial change within weeks, with the most pronounced result evident at 3–6 months after the final session as collagen remodelling continues. It’s not a substitute for surgical liposuction in patients with large volumes of fat to reduce, but for localised stubborn fat plus skin laxity (or cellulite), it produces visible, durable improvements without surgery.
Benefits of TightSculpting
TightSculpting has a clear set of strengths. Below is a realistic summary of what the treatment can achieve.
Most non-surgical body treatments do one or the other. TightSculpting does both in the same session. The Nd:YAG PIANO® mode disrupts subcutaneous fat cells; the Er:YAG SMOOTH® mode tightens the overlying skin. The combined effect addresses the most common patient concern (loose skin appearing after fat reduction) in a single integrated protocol.
TightSculpting uses laser energy applied to the skin surface only. There are no incisions, no needles inserted into tissue, no general anaesthetic. The risk profile is meaningfully lower than surgical — though the result is also less dramatic. For patients seeking moderate body contouring without surgical recovery, this trade-off makes sense.
Most patients return to normal activities the same day. Mild redness and warmth in the treated area for a few hours is the only common side effect. There’s no time off work needed, no compression garments required, no exercise restrictions beyond 24–48 hours.
The Nd:YAG 1064nm wavelength has low absorption in melanin, which means the risk of post-inflammatory hyperpigmentation in darker skin types is significantly lower than with shorter-wavelength lasers. TightSculpting is suitable for Fitzpatrick types 1–6, with energy settings adjusted for darker skin types as standard practice.
TightSculpting can be applied to the abdomen, flanks (love handles), thighs (inner, outer, anterior, and posterior), buttocks, arms, upper back (bra rolls), and submental area (double chin). In a single session, multiple smaller areas can be treated. For larger areas (full abdomen plus flanks, for example), a longer session is booked.
The fat cells disrupted during TightSculpting are eliminated by the body’s lymphatic system over the following weeks and don’t regenerate. The fat reduction is permanent provided you maintain a stable weight — significant weight gain after treatment can cause remaining fat cells to enlarge. The skin tightening result lasts several years with maintenance sessions every 6–12 months.
TightSculpting applied with cellulite-specific protocol settings (using the L-Runner® and T-Runner® scanners) is marketed as OrangeLase™ — Fotona’s branding for cellulite reduction. The combination of fat-cell disruption (PIANO® mode) and surface tightening (SMOOTH® mode) addresses both root causes of cellulite: enlarged fat lobules pushing through weakened connective tissue, and surface skin laxity. See Block 8 for full cellulite-specific guidance.
For patients with significant volumes of fat to reduce, surgical or produces more dramatic results in one session. For pronounced skin laxity (skin aprons, significant redundancy), surgical excision (tummy tuck, arm lift, thigh lift) is the better answer. For deeper cellulite tethering, or may be more appropriate. We will tell you directly at consultation which option suits your case.
Who is Suitable for TightSculpting? Comparison vs Liposuction & Other Options
TightSculpting is suitable for adults aged 18 and over with localised stubborn fat, mild to moderate skin laxity, or cellulite — provided their goals match what non-surgical laser treatment can realistically deliver.
A face-to-face consultation is required before any TightSculpting treatment is booked. The practitioner will examine your treatment area, classify your Fitzpatrick skin type, discuss your medical history, and tell you directly which treatment (or combination) is right for your case. We don’t sell what isn’t appropriate.
The TightSculpting Procedure
A TightSculpting session takes 30 to 60 minutes depending on the size and number of areas treated. The session uses Fotona’s SP Dynamis laser platform with two laser modes applied in sequence to the same treatment area.
Most patients describe the sensation as a deep, gentle warming with brief moments of more focused heat. The Nd:YAG PIANO® mode produces a noticeable warm sensation that builds gradually as the laser is scanned across the area — this is the deep tissue heating doing its work. The Er:YAG SMOOTH® mode produces a lighter, more superficial warming sensation. Most patients tolerate both modes without numbing.
Single-area treatments (e.g. abdomen alone, or thighs alone) take 30–45 minutes. Multi-area treatments (e.g. abdomen plus flanks, or full thighs plus buttocks) take 60–90 minutes. With check-in, photographs, and aftercare advice, allow 60 minutes for a single area or up to two hours for larger combined treatments.
A standard course is 4 to 6 sessions spaced 2 to 4 weeks apart. Most patients see initial change after 2 sessions, with the most pronounced result evident at 3–6 months after the final treatment. Patients with more pronounced concerns may benefit from up to 8 sessions. After the initial course, maintenance sessions every 6 to 12 months help preserve and extend the result.
For cellulite (OrangeLase™ protocol), we typically recommend 3 to 5 sessions spaced 4 weeks apart.
TightSculpting Aftercare and Recovery
TightSculpting recovery in summary: Mild redness and warmth in the treated area for a few hours up to 24 hours is the only common side effect. Most patients return to work and normal activities the same day. No time off work required, no compression garments, no exercise restrictions beyond 24–48 hours.
If you develop persistent redness or swelling beyond 48 hours, signs of infection (increasing redness, warmth, pus, fever), unusual blistering, or any other concerns, contact the clinic on . Side effects beyond mild transient redness and warmth are uncommon but should always be reviewed.
Expected Results from TightSculpting
TightSculpting produces gradual, layered results. Both the fat reduction and the skin tightening develop over weeks to months — this is biological remodelling, not the immediate change of surgical liposuction.
A mild tightening effect is often visible within hours of the first session — this is the immediate collagen contraction response from the Er:YAG SMOOTH® heating. Most of this initial tightening is subtle. Mild redness and warmth resolve within hours.
The fat cells disrupted by the Nd:YAG PIANO® mode are gradually cleared by the body’s lymphatic system over the following weeks. Visible fat reduction begins to appear after 2–3 sessions and continues developing for 3–6 months after the final session. The fat reduction is permanent provided you maintain a stable weight.
The structural skin tightening develops as fibroblasts produce new collagen and elastin in response to the controlled thermal injury from the Er:YAG SMOOTH® mode. This is the biological enhancement that makes TightSculpting results meaningful — the skin gains genuine firmness and improved texture, not just temporary heat-induced contraction. The full benefit is typically visible at 3–6 months after the final session.
A standard course is 4 to 6 sessions spaced 2 to 4 weeks apart. Each session builds on the previous one. Most patients see visible improvement after 2–3 sessions, with the most pronounced result evident 3–6 months after the final session.
The fat reduction is permanent — disrupted fat cells don’t regenerate. However, remaining fat cells in the treated area can enlarge if you gain significant weight after treatment. The skin tightening result lasts 2–3 years on average, though this is influenced by age, lifestyle, and skin condition. Maintenance sessions every 6 to 12 months help preserve the result over time.
TightSculpting produces meaningful, visible body contouring and skin tightening for the right patient — moderate localised fat plus mild-to-moderate skin laxity. It does not replicate the dramatic single-session result of surgical , and it doesn’t repair severe skin redundancy the way surgical excision (tummy tuck, arm lift) does. Patients who come in expecting surgical-level results from non-surgical laser are typically disappointed; patients who come in for moderate body contouring without surgical recovery are usually very pleased.
TightSculpting for Cellulite — The OrangeLase™ Protocol
OrangeLase™ is the cellulite-specific protocol of TightSculpting — using the same Fotona Nd:YAG PIANO® and Er:YAG SMOOTH® laser modes, with treatment parameters tuned specifically for cellulite reduction. It addresses both root causes of cellulite in a single integrated treatment: enlarged fat lobules pushing through weakened connective tissue, and surface skin laxity.
Cellulite is the dimpled, "orange-peel" appearance of skin most commonly seen on the thighs, buttocks, and abdomen. It develops when subcutaneous fat lobules push upward through fibrous bands of connective tissue (fibrous septa) that anchor the skin to deeper structures. As these fibrous bands tighten with age and the fat lobules enlarge, the skin between them is pulled inward, creating the characteristic dimpled texture. Surface skin laxity worsens the visible effect.
Cellulite affects 80–90% of post-pubescent women regardless of body weight. It’s not a weight problem and weight loss alone rarely resolves it.
For more on the differences between cellulite, stubborn fat, and skin laxity, see .
Step 1 — Nd:YAG PIANO® mode (deep fat reduction). The Nd:YAG laser delivers controlled deep heat to the subcutaneous fat layer using the L-Runner® and T-Runner® scanners. This reduces the size of the enlarged fat lobules pushing against the connective tissue. With smaller fat lobules, the dimpling pressure on the overlying skin is reduced.
Step 2 — Er:YAG SMOOTH® mode (surface tightening). The Er:YAG laser in non-ablative SMOOTH® mode then tightens the skin surface, stimulating new collagen and elastin production. The improved skin firmness reduces the visible appearance of remaining dimpling.
The combined effect is meaningful improvement in cellulite appearance — smoother skin texture, reduced dimpling, and firmer overall contour.
A typical cellulite course is 3 to 5 sessions spaced 4 weeks apart. Each session takes 30–45 minutes per area treated. Most patients see initial improvement after 2 sessions, with the most pronounced result evident at 3 months after the final session. Maintenance sessions every 6 to 12 months help preserve the result over time.
OrangeLase™ uses the same pricing structure as standard TightSculpting — see Block 9 below.
No cellulite treatment — laser, surgical, or otherwise — guarantees complete elimination of cellulite. Cellulite is influenced by genetics, hormones, connective tissue structure, and ageing, and these factors don’t fully reverse with treatment. What OrangeLase™ does deliver is a meaningful, visible reduction in the appearance of cellulite — smoother skin, less prominent dimpling, firmer texture — which improves how the affected area looks in clothing, swimwear, and on close inspection.
For patients with deeper, more anchored cellulite dimples — particularly persistent specific dimples that don’t respond to dermal-level treatment — surgical directly cuts the connective tissue tethers. combines RF heating with subcutaneous treatment for moderate to severe cases. We will tell you directly at consultation whether OrangeLase™ is the right answer for your specific cellulite presentation, or whether subcision or CelluTite would suit you better.
TightSculpting Cost in London
TightSculpting at Centre for Surgery starts from £650 per session for a single body area, with course-of-3 packages from £1,750 and course-of-6 from £3,250. The same pricing applies whether you’re booking TightSculpting for fat reduction, skin tightening, or cellulite (OrangeLase™ protocol) — the laser hardware, treatment time, and clinical expertise are identical across indications.
All prices are all-inclusive — covering the practitioner’s fee, treatment room, eye protection, topical numbing if requested, and follow-up review appointments.
A "single body area" is approximately the size of one abdomen, both flanks (love handles), one full thigh region, both arms, the bra-roll area, or the submental (double chin) area.
The 6-session option is recommended for patients with more pronounced concerns or who want the most pronounced cumulative result.
For combined treatments (e.g. abdomen plus flanks, or thighs plus buttocks), an integrated quote is provided at consultation. Multi-area pricing is typically discounted vs booking each area separately. As a guide, two-area treatment is approximately 1.5× single-area pricing rather than 2×.

What To Expect
A face-to-face consultation at our Baker Street clinic with one of our GMC-registered aesthetic practitioners is the first step. The practitioner will examine your treatment area, classify your Fitzpatrick skin type, discuss your medical history, and assess whether TightSculpting is the right answer for your case. For some patients with significant fat volumes, surgical liposuction will give a more dramatic result. For others with deep cellulite tethering, subcision or CelluTite may be more appropriate. We will tell you directly which option suits you best. If TightSculpting is appropriate, the practitioner will recommend a treatment plan — typically a course of 4 to 6 sessions, 2 to 4 weeks apart, or 3 to 5 sessions for cellulite (OrangeLase™ protocol). High-resolution photographs are taken for your records and to compare progress. A mandatory two-week cooling-off period applies before your first treatment is booked. Consultations last around 30 minutes. You are welcome to come back for further consultations free of charge if you have additional questions.
On the day of treatment, the area is cleansed and treatment markings are made. Standard laser safety goggles are worn. Topical numbing cream can be applied for 20 to 30 minutes if you prefer additional comfort, though most patients tolerate the treatment without it. The practitioner then works through two Fotona laser modes in sequence. Step 1 is the Nd:YAG laser in long-pulse PIANO® mode using the L-Runner® or T-Runner® scanner — this delivers controlled deep heat to the subcutaneous fat layer, disrupting fat cells. Step 2 is the Er:YAG laser in non-ablative SMOOTH® mode applied to the same area — this tightens the overlying skin by stimulating collagen contraction and remodelling. Most patients describe the sensation as a deep, gentle warming with brief moments of more focused heat. A single-area session takes 30 to 45 minutes; multi-area sessions take 60 to 90 minutes. Once complete, the skin is cooled with a soothing post-procedure compress and aftercare advice is provided.
TightSculpting is associated with limited downtime. Mild redness and warmth in the treated area for a few hours up to 24 hours is the only common side effect. Most patients return to normal activities the same day. The first 24 hours: stay well-hydrated to support lymphatic clearance of disrupted fat cells. Avoid hot showers, saunas, and steam rooms for 24 hours. Avoid intense exercise for 24 to 48 hours. Mild swelling overnight is uncommon but resolves quickly. Days 2 to 7: resume normal activities including light to moderate exercise. Apply broad-spectrum SPF 30+ to the treated area daily. Avoid aggressive exfoliation, retinoids, or AHAs in the treated area for 5 to 7 days. Avoid direct sun exposure on the treated area without SPF for at least 4 weeks. Optional manual lymphatic drainage massage 24 to 72 hours after each session can support fat clearance. Maintaining a stable weight after the treatment course preserves the long-term result.
A mild tightening effect is often visible within hours of the first session — this is the immediate collagen contraction response from the Er:YAG SMOOTH® heating. Visible fat reduction begins to appear after 2 to 3 sessions and continues developing for 3 to 6 months after the final session. A standard course is 4 to 6 sessions for body contouring, or 3 to 5 sessions for cellulite (OrangeLase™ protocol), all spaced 2 to 4 weeks apart. Most patients see visible improvement after 2 to 3 sessions, with the most pronounced result evident 3 to 6 months after the final session. Maintenance sessions every 6 to 12 months help preserve the result over time. Realistic expectation: TightSculpting produces meaningful body contouring and skin tightening for moderate localised fat plus mild-to-moderate skin laxity. The fat reduction is permanent provided you maintain a stable weight. The skin tightening result lasts 2 to 3 years with maintenance. It does not replicate the dramatic single-session result of surgical liposuction; for that, surgery is the appropriate answer.
